RETIREMENT ANNOUNCEMENT

Dear Patients:
This serves as formal notice that I will retire from clinical practice as of April 26, 2026.
It has been a great pleasure providing for your health care needs over the years, and it is not easy for me to give it up.
As of April 27, 2026, Dr. John Cottingham will take over my practice.
Of course, you may seek medical care from another doctor if you like. If you choose to do so, I recommend looking for a new physician as soon as possible.
Your medical records are confidential, and a copy can be transferred to another doctor or released to you or to another person you designate only with your permission. If you plan to continue with Dr Cottingham, you need to sign an authorization to release your files to his practice. If you choose to see a different physician, please sign the enclosed authorization and return it to my office as quickly as possible so we may transfer your records to your new doctor. After April 26, 2026, your records will be maintained by Dr Cottingham’s office. His office is located at 400 W. Medical Center Blvd Suite 215, Webster, TX 77598 Telephone: 281 612 1323.

This direct primary care practice model provides high quality medical care with extra time between patient and physician to discuss all health care issues, along with an increased emphasis on disease prevention and wellness.

All patients in the practice pay an annual fee. The annual fee includes an annual Comprehensive Wellness Assessment, a secure patient web portal, an online personal health record and ongoing medical care throughout the year. Payments may be made annually or monthly. In essence, patients engage me as their personal private physician. Our goal is to serve your medical needs with an unmatched level of quality, value, and convenience.
We provide care for patients who desire excellence in traditional medicine and want guidance regarding the protection of their most precious asset – their health.
Clear Lake Internal Medicine does not file insurance or Medicare for its professional services. Patients may use their insurance for services provided by hospitals, labs, and specialists who accept insurance or Medicare.
